The video below shows a precast, prestressed double-tee (DT) flange failure at a parking structure on the campus of California University of Pennsylvania in August of 2016. The flange was reinforced with a carbon-fiber-reinforced-polymer (CFRP) grid. The failure happened as students were moving into their dorms. The first two shots are from above where a minivan drove over the cracked DT flange. The last shot is from below where two people were nearly hit by the falling debris.
